First Visit

You are encouraged to complete the registration and medical history forms on line. It will save time at your first visit.

When you call, our offices will schedule your visit in a timely manner and our Doctors will attempt to see you promptly when you arrive. A referral from another doctor's office is encouraged but not necessary. Please assist us, if possible, by providing the following at the time of your first visit:

river boat at Georgia
  • Your surgical referral slip and x-rays. If x-rays are not available, current or diagnostic they can be taken at our office.
  • A list of medications you are currently taking.
  • Medical & dental insurance information or completed forms.

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office.

IMPORTANT: All patients under 18 years of age must be accompanied by a parent or guardian.

Your initial appointment will consist of an examination and consultation. We appreciate and require your input regarding the nature, severity, extent, duration, etc. of your problem. Also, please alert us if you have a medical condition that may be of concern prior to surgery. Most importantly, your doctor will thoroughly discuss your diagnosis, treatment options, the planned surgical procedure and the appropriate anesthetic so you will know exactly what to expect the day of your surgery. This will allow you to make an informed decision/consent about your planned surgery. As is common practice with every medical procedure you will be required to sign a surgical consent form before your surgery.

In some cases, particularly simple procedures or emergencies, surgery can or must be performed on your initial visit. These cases are the exception however and usually a complex medical history or treatment plan will require a second appointment to provide treatment. Insurance restrictions may also delay treatment as many insurance benefits may require.

Since our practice frequently treats hospital emergency room and outpatient emergencies we do experience scheduling priorities. Every effort is made to keep the schedule on time however there are instances when the best planning cannot control your waiting room time. Occasionally major hospital trauma cases necessitate cancellation of scheduled appointments. Please try to understand and bear with us. If this occurs, you will be notified and we will be happy to reschedule you at a time convenient to you.
